We do not require a length of term contract, however we do request that you sign our Policy Contract as an agreement to the terms and conditions outlined on the this Policies page of this website. 

Once you have scheduled an appointment for your first cleaning, please print out this page using the "Click for Printer-Friendly Version" button below. Fill out the contract information and fax the entire page to us at the fax number below. Doing this will activate your account and we can apply the coupon below to the bill on your first time cleaning. 

Please have this info in to us well before your service day so we can have your account activated in time. Preferably within 48 hours of actually making the appointment on the phone.

If you do not have access to a fax machine, you may copy & paste this page into an email and fill out the information and email it to the address below.  We will then print it on our end for your file. Please watch your return email to be sure you don't receive an undeliverable message on the email. We sometimes have massive amounts of spam blocking our email account. If you need to mail the contract in, see below for the mailing address. By far, the best method is by fax. Thank you.
